113學年度第2學期應屆畢業生確認英文姓名須知

(English name conformation for Graduates )

1. 本校學位證書為中、英文版。大學部及研究所應屆畢業生請即日起至2025年5月17日止上網確認英文姓名，以利製作學位證書。
   填寫路徑：https://ap.kh.usc.edu.tw/sso/login.aspx→輸入學號及密碼→教務資訊模組→教務資訊系統→學籍資訊→〔SB0110〕英文姓名確認→按下「確認」鈕。
2. 英文姓名請依護照上之姓名詳實填寫(含英文字母大小寫及符號標示方式)；無護照者，請至外交部領事事務局網頁查詢。

3. 應屆畢業生請於規定時間內完成〔SB0110〕英文姓名確認，未完成英文姓名確認者將影響線上離校申請作業。
4. 逾期確認或經個人確認且畢業證書印製完成，始發現有誤者，將由同學自行付費200元，申請補發學位證明書(原證書作廢)。

Please complete in the Administration System the confirmation of your English name by May 17, 2025. Your English name should be identical with the one on your passport. Students shall not ask for modification after the deadline, nor shall students request for remaking the degree certificates. Students who fail to complete the confirmation before deadline will not be able to request leaving school online.
